Patients - Registering with the State

Registration with the state is no longer mandatory. All it takes is a copy of your physician's certification on tamper proof paper, which you can download below.

Download the physician's certification - Take this to your doctor, have it filled out and signed on tamper proof paper. This provides the same legal protections as a regisration card.

Download the Caregiver designation form - If you do not register with the state, this is the form to designate a caregiver.

Download the state application - If you wish to register with the state, this is the correct form to register.
